• Complete all sections of these Forms.
• Please sign these printouts.
• Fax the signed forms to Grace Electronics Ltd on 01252 667799, along with a copy of your business letterhead, certificate of incorporation (where applicable) and a copy of your company VAT registration certificate.
• Send originals in the post.
